Initial Visit

On your first visit to Sunflower Health & Wellness Consultants please bring your insurance card, photo identification and a referral if required. Have your emergency contact information available. Please do not bring small children to your appointment as we have sessions with clients that require a quiet environment. Arrive 15 minutes early or complete the new patient forms and bring those with you. You will have a couple of additional forms to complete upon arrival.
